How rTMS Works

rTMS uses targeted magnetic pulses to activate modulate neuronal pathways that cause mental illness. This repeated activation:

  • Helps restore balance in brain circuits.
  • Increases production of brain-derived neurotrophic factor (BDNF), promoting neurogenesis and neuroplasticity.
  • Improves symptoms of depression, anxiety, and other disorders.

Unlike electroconvulsive therapy (ECT), rTMS does not require anesthesia, and patients remain fully awake and alert during treatment.

Conditions Treated with rTMS

rTMS is FDA-approved for:

  • Major Depressive Disorder (MDD): After at least one failed antidepressant trial.
  • Obsessive-Compulsive Disorder (OCD): As an adjunct to ERP therapy and pharmacotherapy.

It is also effective for:

  • Generalized Anxiety Disorder (GAD)
  • Post-Traumatic Stress Disorder (PTSD)
  • Depression secondary to Traumatic Brain Injury or Concussion
  • Tinnitus and Nicotine Use Disorder

Note: Treatments for anxiety disorders and PTSD are currently considered off-label.

What to Expect During Treatment

  • Typical Protocol: 5 sessions per week for 6 weeks (30 treatments total).
  • Accelerated Protocol: 30-50 treatments over 1-3 weeks using intermittent theta-burst stimulation (iTBS).
  • Treatment Duration: 3.5 to 30 minutes, depending on the protocol.
  • Tapering & Maintenance: Recommended based on individual needs.

Patients can drive themselves home after sessions and resume normal activities immediately.

Eligibility Criteria

To ensure safety and effectiveness, patients are to meet the following criteria:

  • Diagnosis of MDD, PTSD, OCD, GAD, Tinnitus or Nicotine Use Disorder.
  • No non-removable metallic hardware in the head (dental hardware is acceptable).
  • Not acutely suicidal (requires a higher level of care).
  • No severe active substance use issues.
  • Symptoms not primarily caused by substance use.

Treatment Includes:

  • rTMS treatments,
  • Full psychiatric assessment with comprehensive treatment plan
  • 12-months follow-up with psychiatrist for medication management
  • Psychotherapy every 4-6 weeks.

Coverage & Payment Information

  • rTMS is not covered by Alberta Health Insurance.
  • Dr. Comeau is a registered provider with Veteran’s Affairs (VA).
  • Some disability and private insurance plans may cover treatment. Please check with your provider.
  • Covered for eligible patients through Veterans Affairs, AISH and WCB. Other insurance providers may cover on a case-by-case basis. Please check with your insurance provider to see if you are eligible

There is a $250 non-refundable deposit required prior to the rTMS consultation, which can be applied towards your treatments
